Medical Consent and Release of Liability (please initial)

I, the undersigned parent or guardian of this student, a minor, do hereby authorize the directors and teachers of Northwest Youth Theatre (NYT) as agents for the undersigned to consent to medical treatment in an emergency. I hereby release and discharge NYT from any and all claims for personal injuries.

I agree that photographs of this student taken during workshop hours may be used for promotional purposes by NYT.

Payment Information

Tuition for each term is $135.00 per term, payable in three installments of $45.00. Receive a discount for pre-paying the full amount by the first rehearsal ($120.00 for the term).

Holders of the Passport Unlimited Employee Discount Card will receive 20% off total tuition for the term. Register online and enter promotional code PASSPORT. You will need to present your Discount Card on the first day of classes to qualify for the discount.

Discounts cannot be combined. We will apply which ever discount is the greatest for your family.

To secure enrollment in any class or camp, please send non-refundable $20 deposit for each class. The remaining balance for the first month's tuition (or full camp tuition) will be due the first day of class. On the following page you will also have the option to pay in full.

REFUND POLICY: Full refund if class is cancelled by NYT. Tuition, less deposit, will be refunded up to second class session.
